Overview of Ohio Immunization Summary for School Attendance:
- KG students must submit immunization records.
- 7thGraders are required to have the DTap/DT Tdap/Td (Diphtheria, Tetanus, Pertussis) vaccination and first dose of Meningococcal (MCV4) vaccination.
- 12thGraders must have the second dose of the Meningococcal (MCV4) vaccination

Ohio Revised Code, Section 3701.13 and 3313.671, mandates that every child be properly immunized in order to attend school. Exemption forms must be submitted each school year. Ohio State law allows a 14-school day period for you to provide a record of your child's immunizations before being excluded from school.
Where to Send Vaccination Records:
After your student has been vaccinated, please send records to their respective building nurse. You may email records or have your student’s physician fax records directly to the building. Contact information for each building is below:
